Appointed by local officials in the six counties that make up our service area, the Community Advisory Council serves as the bridge between Health Forward and the communities we serve.
The Community Advisory Council’s role in moving our mission forward in a meaningful way includes nominating individuals to serve on Health Forward’s Board of Directors, identifying community health concerns, and publishing an annual review of our work.
Each year, the Community Advisory Council assesses Health Forward Foundation’s body of work and its community impact.

The Community Advisory Council exists as a 501(c)(3) nonprofit entity entirely separate from Health Forward Foundation. Donations can be made to the council through PayPal.
